Mid-West Toronto Family Practice Network

In the Mid-West Toronto Region, a network of family practitioners have come together to improve and coordinate primary care through the healthcare system. While the information below may be of interest to the public, it is written to inform primary care providers of the benefits of the Network.

The Mid-West Toronto Family Practice Network (MWT-FPN) is a non-profit corporation that represents all interested family practitioners who practice in the Mid-West Toronto region. 

Membership is voluntary and open to any family practitioner in the Mid-West Toronto region Membership in the MWT-FPN is not legally binding and members can withdraw at any time. Members are kept informed of and have the opportunity to influence change affecting primary care in the region– whether these concern new services, access to resources, or plans or policies that are being contemplated by the Mid-West Toronto Ontario Health Team.

  • Membership Benefits and Rights

    • The opportunity to influence decision-making regarding primary care in the Mid-West Toronto Region

    • Receive communication updates regarding relevant initiatives in the Mid-West Toronto region, including COVID related supports

    • Receive notices of membership meetings, to be held at least once a year

    • Elect the directors of the corporation

    • Receive annual financial statements of the Family Practice Network

    • Confirm the by-laws and any amendments

    • Appoint the auditors

    • Approve special matters such as a change of name, change of mandate, or amalgamation with another organization, as set out in the Corporations Act (Ontario)

One size does not fit all. This video showcases the diversity of primary care models in our region. We highlight just some of the ways we work to take care of our patients, particularly those who are vulnerable and come from marginalized communities.

The MWT-FPN has a Board of Directors that inclusively represents all practice models; Family Health Teams (FHTs), Family Health Organizations (FHOs) Community Health Centres (CHCs), small practices, Family Health Groups (FHGs), Comprehensive Care Models (CCMs) and Fee for service solo physicians. As we embark on our second year, we continue to refine our goals and also seek further representation from members in our region and on our Board of Directors. Please note we are funded to offer stipends to people who participate.


Goals for Year 2

  • Grow the membership of family practitioners who practice in the Mid-West Region

  • Design and implement a feedback mechanism to elicit input from our broader membership

  • Enhance services for homebound patients of our practicing Mid-West FPN members

  • Recruit new members for the BOD and working groups

  • Support the spread of digital technology (on-line booking/e-referral)

  • Disseminate information to improve access to mental health resources

  • Develop a primary care/specialist collaborative to address improving coordination and patient co-management

  • Inform priorities and contribute to the planning and implementation of Mid-West Toronto Region initiatives

Annual General Meeting Summary

Current News

We were pleased to host our second Annual General Meeting at the Faculty Club, University of Toronto, on June 7, 2023. A total of forty-eight members atended in person or virtually: the majority partaking of wine and canapés on the patio during the first hour of the meeting.

Our business meeting began with our co-chairs, Michelle Naimer and Pauline Pariser, giving the year in review, emphasizing the strides we have made in onboarding members to On-Line Appointment Booking and OCEAN e-referral, growth of programs supporting people with mental health challenges and homebound older adults and the launch of our primary care/specialist collaborative aiming to improve communication and coordination between our two sectors.Recent opportunities to enhance management of Congestive Heart Failure and Limb Preservation for people at risk for lower limb amputation were presented, as well as a description of the efforts that were made to inform the recent Expression of Interest to fund a range of positions that will build team-based care in Mid-West Toronto.
