    The IMAGINE Clinic and Remembering What I Love About Family Medicine

    After 30+ years in community-based family medicine in midtown Toronto, I was looking for something a little different. Like so many of us emerging from the Covid pandemic, I was feeling burned out. I happened to read an email from the OMA and saw a post from the IMAGINE clinic recruiting preceptors. IMAGINE was created and is run by University of Toronto medical students to offer walk-in medical services to disenfranchised patients in downtown Toronto.

    Primary Care Specialist Collaborative

    The Primary Care Specialist Collaborative, which launched in May, emerged as a direct response to your requests for enhanced interaction and coordination between primary care and specialists.

    The collaborative has welcomed three patient partners into its ranks, amplifying the voices involved in the ongoing efforts. This brings the total membership to six family physicians, six specialists, and three patient partners.

  • Digital Update

    Join Ontario's funded eReferral & eConsult Programs

    Our region is working to get more specialists onboarded to eReferral so we can easily refer patients to specialists from within our EMRs (a web-based version is available as well for EMRs that do not support eReferral integration). Many family doctors in our region have already signed on and are eagerly awaiting more specialist colleagues to sign on.

    Good news! More services have been added to the eReferral HealthMap including Central intake for CAMH, Diagnostic Imaging from the Joint Department Medical Imaging (JDMI- UHN, Women's College Hospital, & Sinai Health), the Women's College Breast Centre, and more!

  • Mid-West Family Practice Network Annual General Meeting

    We had a successful 2nd Annual General Meeting in June, 2023. It was great to mingle with family physicians and nurse practitioners from the area prior to the meeting, and have break out groups to discuss a funding proposal the MWT-OHT in collaboration with the MWT-FPN submitted to Ontario Health to improve access to team-based care for community family physicians.

  • Update on the Lower Limb Preservation Demonstration Project

    Since November 2020, the partner organizations of the Mid-West Toronto region – health care and community service providers, have been maximizing existing partnerships and working to create an Ontario Health Team (OHT) that truly serves our communities. The Mid-West Toronto OHT integrates health and community care services to allow smooth patient navigation for a disease or condition.
