Your Ontario Health Team

In 2019, the Government of Ontario announced the creation of Ontario Health Teams (OHT) that partners health care providers in the same geographic region to better connect patients and providers in their community.

On November 18, 2020, the partner organizations of the Mid-West Toronto region have been approved to become an Ontario Health Team by the Ministry of Health, marking a significant milestone for patients, clients, families and caregivers in downtown Toronto’s west-end.  Click here to read our full application to the Ministry of Health.

Since then, health care and community service providers in Mid-West Toronto have been building off our existing partnerships and working to create an OHT that truly serves our communities. We all share the belief that if we design a system the works for the most vulnerable in our community, we are creating a system that works for everyone. 

Our Team has had the privilege of having the support of people with lived experience provide their perspective every step of the way. By having our patients and clients at the table with clinicians, system planners and agencies, we are able to take a 360-degree approach to planning.

Our hope is that, when completed, all patients and clients in the Mid-West Toronto Ontario Health Team region will find it easier to get the care and services they need.

By connecting hospitals, family doctors, community health centres, home care, mental health and addictions services, and social services in our region, we are taking the burden off our patients and caregivers and creating a more seamless health care experience.
