As our Team’s work continues to ramp up, we are creating a number of strategies, protocols and plans that are intended to guide our efforts in an equitable and meaningful way. We created the guides collaboratively with input from Partners across the Ontario Health Team.
Patient and Caregiver Engagement
The Partners of the Mid-West Toronto Ontario Health Team believe that we cannot reimagine our healthcare system without meaningfully engaging our patients, clients, families and caregivers in our planning. Our Patient and Caregiver Engagement Strategy provides an overview of how we hope to partner with the people we serve in our efforts to improve care.
Communicating with Primary Care
We are privileged to have more than 250 Primary Care Partners who work in our region. Keeping this important group of stakeholders informed and engaged in the work of the Ontario Health Team is vitally important. This communications protocol outlines how we intend on engaging our primary care colleagues.
