Attaching patients to primary care:  

We are helping to organize streamlined attachment of patients to new practices.  We especially want to help family physicians new to practice to have a balanced practice of people with straightforward concerns and those with more chronic conditions and to provide support to help manage people living with more complex medical and social challenges. Please let us know if you or colleagues you know are setting up practice as part of the Ministry Income Stabilization Program.


Support for your homebound patients :

In a continued quest to provide our community practices with access to an inter-professional team, our Primary Care at HomeWorking Group has endorsed a proposal that will be jointly submitted to expand the Independence at Home Outreach team. If this is funded, we will deliver a dedicated pathway for our SCOPE family practices.

Remote Care Monitoring Program:

We have heard from many of you how difficult it has been to provide primary care to patients who, because of frailty, immobility, mental health challenges, have been unable to attend in-office appointments.

The Remote Care Monitoring Program is free of charge,monitored 24/7 and can measure activities of daily living such as nutrition, risks for older adults such as pressure sores, wandering, medication administration and can also connect you to accessible specialist care at the Salvation Army Toronto Grace Hospital.

Safety alerts are sent to the program’s 24/7 call centre and ensure all concerns are escalated to the appropriate service provider. In addition the RCM program offers access to the following Toronto Grace. specialists: Geriatrician, Respirologist, Pulmonary Rehab, Rheumatologist, Chiropodist and Internal Medicine Support.
