Across Mid-West Toronto, family practices, community health partners, and family health teams are working together to improve how people access primary care. As part of this effort, some positions posted by our Partner organizations are connected to a team-based primary care initiative. This initiative is led by the Mid-West Toronto Ontario Health Team (MWT-OHT) and the Mid-West Toronto Family Practice Network (MWT-FPN).
Together, we are focusing on strengthening primary care attachment, helping more people in our region build lasting connections with family doctors, nurse practitioners, and interprofessional care teams.
