About the
Mid-West Toronto Ontario Health Team
In 2019, the Government of Ontario announced the creation of Ontario Health Teams (OHT) that partners health care providers in the same geographic region to better connect patients and providers in their community.
Since then, health care and community service providers in Mid-West Toronto and our Patient and Caregiver Partners, have been building on existing partnerships to create an OHT that truly serves our communities. We share the belief that if we design a system that works for the most vulnerable in our community, we are creating a system that works for everyone.
By connecting hospitals, family doctors, community health centres, home care, mental health and addictions services, and social services in our region, we are taking the burden off our patients and caregivers and creating a more seamless health care experience.
Our Partners
One of the most complex OHTs in the province, our Partnership is comprised of Patient and Caregiver Partners, community support service organizations, community mental health and substance use organizations, hospitals, a long-term care home, community health centres and nearly 400 primary care providers.
