Looking to be connected to primary care? The first step is to join Health Care Connect to be matched with a local family doctor, nurse practitioner, or primary care team accepting new patients in your community.

You can register yourself or your whole family online at ontario.ca/healthcareconnect or by phone at 811.

Primary Care Attachment Liaison

Good news! A primary care provider in your area is ready to accept you as a new patient. To help onboard you to their clinic, the Primary Care Attachment Liaison will contact you to schedule a brief intake call to collect your medical and social history, including immunization records, completed cancer screenings, and more.

Following the intake call, the Liaison will help set up your profile with your new family doctor and schedule your first appointment with them.
