Access to Mental Health Services (see below for some options and stay tuned for more to come)
Improved home-care provision – (starting to address this issue with a dedicated working group)
Especially for frail elderly: includes post-discharge follow-up, streamlined palliative care (i.e. one centralized form), coverage for in-home labs, support for people with dementia.Specialist Access – (stay tuned for streamlined pathways and improvements)
i.e. more centralized referral pathways, specialist inventory including availability, area of expertise, improved communication and coordination between specialty and primary care

Access to Information Technology ( e-referral, hypercare app, shared EMRs across sectors)

Continued Virtual Care funding
Primary Care access support for people without OHIP coverage
